Register online

The online registration form collects your details, a brief outline of your medical history and a record your preferences. This information may be added to your record and will allow us to begin to provide you with medical services.

Please complete one form for each family member.

Temporary registration

You will need to download and complete the GMS3 form and send it to our Registration team through email at: [email protected]

Upon completion of your registration, we will process your request within 48-72 working-day hours. However, in the event of a high volume of registrations, the processing time may be extended. Once completed, we will send you a confirmation text. If you require an urgent appointment and are not yet registered, please contact our practice to book a same-day telephone appointment and inform us that you have commenced the registration process.

For urgent medical needs, we will see you on an immediate necessary basis. If you require urgent medication, please arrange this with your previous practice. If this is not possible, please send us a list of your medication via email or post so that we can process your request.

You will be assigned to a doctor who will be your Named Accountable GP. 

You will be added to the practice list, not an individual doctor. You can ask to be seen by any of the doctors in the surgery but it will not always be possible to see the doctor of your choice on a particular day or time.

Your NHS number is a 10 digit number, like 485 777 3456. You do not need to know your NHS number to use NHS services, but it can be useful to have it.

When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.

Your medical records remain with your registered GP but we will provide you with full medical care while you are in the area. If you leave the UK for a period of more than three months the doctor is no longer responsible for your medical care and may not provide repeat medication in your absence.

You can register with us if you are homeless or of no fixed abode but reside within the practice boundary. Further information can be found on the NHS website.

From 2020, adults in England will be considered potential donors unless they choose to opt out or are excluded. For more information please visit the Organ Donation NHS UK where you can also register your decision.

If you are ill while away from home or if you are not registered with a doctor but need to see one, you can receive emergency treatment from our Practice for up to 14 days. After 14 days, you will need to register as a temporary or permanent patient. Contact reception for further information.
