REGISTER WITH THE PRACTICE?

Patients who live within the Practice catchment area (see Practice Area) are registered with the Practice, not an individual GP. Your medical card will be issued in the name of one of the Doctors who will supervise your care. However, you can, at any time, express a preference for a practitioner, for either or all of your medical needs or on a case-by-case basis. This preference will be recorded on your medical notes. Whilst the Practice will always try to comply with your requests on all occasions, it may not always be possible.

In order to fully register with the Practice all patients must:

• Live within the published Practice area
• Provide their medical card (FP4) duly signed
Or
• Complete an application to register form (GMS1 available at reception)
• Attend a health screening appointment with a Practice Nurse/Healthcare Assistant
